Solutions – East Toronto’s Health Collaborative
A voluntary network of healthcare organizations working together to create a coherent, integrated, coordinated and appropriately funded system of care and support for the people of South East Toronto.
Goals of the Solutions’ Network:
- leverage the knowledge, skills, resources and capacities of a broad range of home and community care, health and social service agencies;
- facilitate improvements in service delivery across the continuum of care;
- ease transitions between health care sectors for clients (from hospitals to home), particularly those who are vulnerable;
- improve coordination across the system; and,
- promote better use of scare health system resources.
Solutions is an example of hospital and community health providers collaborating to use their shared knowledge, skills and capacities in order to provide a continuum of services for vulnerable populations in their shared catchment area. Solutions will continue to develop innovative projects that make the best use of available resources to maximize integrated care for individuals and bring benefits to the broader health system.
