*Event Details*

The Canadian Research Network for Care in the Community (CRNCC) and

the Ontario Community Support Association (OCSA) will feature top

speakers from Canada and Europe to learn more about what works, what

does not, and what needs to be done at the local level to implement

successful integrated care initiatives.

 

This symposium will also explore the links between integrated care and

supportive housing, and build on some of the key themes from the CRNCC

- OCSA Supportive Housing 2007 symposium.

 

Janet Lum (Ryerson University and CRNCC, ON): will set the stage by

outlining the general trends in integrated care systems and aging at

home strategies.

 

 

*International Perspective*

Kai Leichsenring (European Centre for Social Welfare Policy &

Research, Austria) : will outline the political, structural, and

organizational conditions that are crucial for best practice models of

integrated care based on the experience from nine European countries.

 

Jenny Billings (Centre for Health Service Studies, University of Kent,

UK): will examine what evidence-based practice elements can be

developed into indicators, the many problems around measuring

outcomes, and what can be considered 'good outcome measures'.

 

Henk Nies (Vilans, the Centre of Expertise for Long-Term Care, the

Netherlands): will present on the Netherlands?s user-driven regional

plans that were developed and now being implemented, and how

integrated services are being pushed onto the policy agenda.

 

 

*Bringing it Home*

Marcus Hollander (Hollander Analytical Services Ltd, BC) & David

Pedlar (Veterans Affairs Canada, PEI): will explore the balance of

evidence regarding continuing care initiatives in Canada and the

policy and practice implications for continuing care when older people

want to stay at home.

 

A. Paul Williams (University of Toronto & CRNCC): will explain the

Balance of Care (BoC) research projects in five different regions of

Ontario that support the argument for safe and cost-effective access

to appropriate home and community care packages as an alternative to

long-term care home placements.
