Solutions - East Toronto’s Health Collaborative is a voluntary network of health care organizations working together since 2001 to create a coherent, integrated, coordinated and appropriately funded system of care and support for the people of South East Toronto.
Solutions Goals:
- leverage the knowledge, skills, resources and capacities of a broad range of home and community care, health and social service agencies;
- facilitate improvements in service delivery across the continuum of care;
- ease transitions between health care sectors for clients (from hospitals to home), particularly those who are vulnerable;
- improve coordination across the system; and,
- promote better use of scare health system resources.
