Award-winning Community Referrals by EMS (CREMS) Project has expanded LHIN-wide in the GTA. This Solutions pilot project launched in the Spring of 2006 and received the first ever Minister's Award in Innovation for Meeting Community Needs through Integrated Care in 2006 in Ontario.
